Forms allow you to collect additional information from attendees during the registration process. Medtribe supports two types of forms: Application Forms for screening attendees, and Booking Forms for collecting information at checkout.

Understanding Form Types

Creating an Application Form

Application forms are used when you need to screen attendees before allowing them to book. This is useful for events with prerequisites, limited places, or specific eligibility criteria.

  1. Open your event and navigate to Forms
  2. Click Add Application Form
  3. Choose to create from scratch or use a template
  4. Give your form a name (for internal reference only)
  5. Add your questions using the form builder
  6. Save the form

Question Types

You can add various question types to your forms:

Required Questions
Mark questions as required to ensure attendees must answer them before submitting. Use this for essential information you need from every applicant.

Reviewing Applications

When attendees submit an application:

  1. You receive a notification
  2. Go to Forms → View Responses
  3. Review each application
  4. Click Approve or Reject
  5. Approved applicants can proceed to book
  6. Rejected applicants receive a notification

Creating a Booking Form

Booking forms collect additional information from attendees during checkout. Unlike application forms, they don't require approval.

  1. Open your event and navigate to Forms
  2. Click Add Booking Form
  3. Choose to create from scratch or use a template
  4. Give your form a name
  5. Add your questions
  6. Save the form

Templates save time by allowing you to reuse form structures across multiple events.

Creating a Template

  1. Go to Templates in the main navigation
  2. Select Application Forms or Booking Forms
  3. Click Create Template
  4. Build your form with questions
  5. Save the template

Using a Template

When creating a new form for an event:

  1. Click Add Application Form or Add Booking Form
  2. Select From Template
  3. Choose your template
  4. The form is created with all template questions
  5. Customise if needed, or use as-is

Managing Templates

From the Templates section, you can:

Viewing Form Responses

To view responses submitted through your forms:

  1. Open your event
  2. Navigate to Forms
  3. Find the form and click View Responses
  4. See all submissions with response details
  5. Filter by session or ticket type if needed

Response Statistics

For each form, you can see:

Duplicate Create a copy of the form
Delete Remove the form (only if editable)

Editing Limitations
Once a form has received submissions, you cannot edit the questions. This ensures data integrity. If you need to change questions, duplicate the form and make changes to the copy.

Common Questions

Can I have multiple forms on one event?
Yes. You can have multiple booking forms and multiple application forms on a single event. Each form operates independently.
Can attendees save and return to an application?
Currently, applications must be completed in one session. Attendees should gather all required information before starting.
How do I export form responses?
Form responses can be exported via the reporting features. Go to your event, select the form, and use the export options.
What happens if I reject an application?
Rejected applicants receive a notification and cannot book for that event. You can provide a reason for the rejection if needed.
Can I make forms optional?
Booking forms are always shown during checkout but individual questions can be marked as optional. Application forms, when attached to an event, require completion before booking.
